Neu und zunächst zögerlich war dann auch der staatliche Zugriff auf diesen umstrittenen Bereich des arbeitsgesellschaftlichen Geschehens, über die Kommunen hinaus durch die einzelnen deutschen Staaten, bis das Deutsche Reich in der Weimarer Zeit zentraler Akteur der staatlichen Arbeitsmarktverwaltung wurde, in der 1927 die Arbeitsvermittlung mit der nun gesetzlich eingeführten Arbeitslosenversicherung institutionell verbunden wurde. Auch der Begriff `Arbeitslosigkeit` hat einen historischen Entstehungs- und Institutionalisierungsprozess, der in den größeren europäischen Ländern in unterschiedlichen kategorialen Zuordnungen erfolgte, die noch gegenwärtig eine gemeinsame europäische Erfassung erschweren (siehe Zimmermann 1996). Die hier vorgelegte Studie zielte darauf ab, die langfristige Effektivität des kommerziellen Ernährungsprogramms metabolic balance ® unter Alltagsbedingungen (d.h. nicht im klinischen Setting) zu untersuchen. Methodik: Prospektive Beobachtungsstudie ohne Kontrollgruppe mit Nacherhebungen nach einem, drei, sechs und zwölf Monaten (Prüfung von `effectiveness` statt `efficacy`). Es wurden sowohl Klienten als auch ihre Betreuer mittels Fragebogenerhebung befragt. Bei den Studienteilnehmern wurden außerdem zu allen Erhebungszeitpunkten Blutproben entnommen, die in einem zentralen Labor analysiert wurden. Studienstichprobe: Im Erhebungszeitraum gaben N=851 Personen ihre Einwilligung zur Teilnahme an der Studie. Nach 12 Monaten waren noch 524 Teilnehmer in der Studie verblieben (drop-out-Quote = 38,4%). 84,1% der Studienteilnehmer waren Frauen. Der mittlere BMI bei Programmbeginn betrug 30,3 (SD=5,7). Bei den drop-out Analysen zeigten sich auf einigen Parametern signifikante Unterschiede, die größenordnungsmäßig allerdings eher gering ausfielen. Die Studienstichprobe entspricht im Hinblick auf Alter, Geschlecht und Eingangs-BMI mit nur geringen Einschränkungen der Gesamtheit aller Klienten, die im Erhebungszeitraum das Ernährungsprogramm in Deutschland begonnen haben (N=30.364). Ergebnisse: Nach 12 Monaten hatten 62,5% der Studienteilnehmer ihr Anfangsgewicht um mindestens 5 Prozent reduziert 31,1% hatten mehr als 10 Prozent abgenommen. Die mittlere Gewichtsabnahme betrug 6,8 kg (SD=7,1). Zwischen Gewichtsabnahme und Programm-Adhärenz zeigte sich es eine klare positive lineare Korrelation. Triglyzeride, Gesamtcholesterin und LDL-Cholesterin waren bei der 12-Monats-Katamnese gegenüber den Eingangswerten signifikant verbessert. Während zu Beginn des Programms 14,2% der Klienten die Symptome eines metabolischen Syndroms aufwiesen, traf dies nach 12 Monaten nur noch auf 3,9% zu. Die gesundheitsbezogene Lebensqualität (v.a. im Bereich psychischer Belastungen) verbesserte sich signifikant mit mittleren bis hohen Effektstärken. Schlussfolgerung: Die Teilnahme an dem untersuchten Ernährungsprogramm führte zu nachweisbaren Verbesserungen des Gesundheitsstatus und der gesundheitsbezogenen Lebensqualität. The author employs a discursive approach to the understanding of identity, which pays attention to the content, context and function of constructions and their identity implications for speakers. The talk articulated in focus groups sessions is characterized by variation as regards constructions of national identity and otherness, by flexibility in the use of resources in this process and by a concern with accountability. The research shows that lay actors` talk about national identity and migration is realised into the context of Western nationalism ideology as articulated in the perspectives of banal and methodological nationalism.
